Compulsory Medical Insurance (CMI) Programs at the Nehama Sanatorium

Originals and copies of the following patient documents:

• Document proving the patient’s identity;
• The patient’s compulsory medical insurance policy;
• Insurance pension certificate (SNILS);
• Referral for hospitalization for specialized medical care under the compulsory medical insurance program in the established form (form 057-u) from the medical organization where the patient is undergoing diagnosis and treatment as part of primary health care.
  The referral must be certified by the personal signature of the attending physician, the personal signature of the head of the medical organization (authorized person), the seal of the attending physician, the seal of the referring medical organization, and contain the following information:
  
  • The patient’s surname, first name, patronymic (if any), date of birth, address of registration at the place of residence (place of stay);
  • Compulsory medical insurance policy number and name of the insurance medical organization;
  • Compulsory pension insurance certificate (if available);
  • Results of laboratory, instrumental, and other types of diagnostics confirming the established diagnosis and the presence of medical indications for the provision of specialized medical care, namely:
    
    • OAK/OAM,
    • RW,
    • HIV,
    • HEPATITIS,
    • BIOCHEMICAL ANALYSIS
      OF BLOOD,
    • ECG,
    • FLUOROGRAPHY,
    • GYNECOLOGICAL EXAMINATION (for women).
  • Profile of specialized medical care indicated for the patient and conditions for its provision (inpatient);
  • Name of the medical organization to which the patient is referred for specialized medical care (from among those participating in the implementation of the territorial compulsory medical insurance program);
  • Surname, first name, patronymic (if any) and position of the attending physician, contact telephone number (if any), e-mail address (if any);
  • An extract from the medical records, certified by the personal signature of the attending physician, the personal signature of the head (authorized person) of the referring medical organization, containing the diagnosis of the disease (condition), ICD-10 diagnosis code, information about the patient’s health, diagnostics and treatment performed, recommendations on the need for specialized medical care, indicating the form of its provision (planned).
